NOTE: To request a Jury Trial, please complete the Docket Control Order Form (also located in the Attorney Section). Once all dates are approved by all parties, please submit the agreed form to the Coordinator's Office for the Judges approval.

** Please be advised, if the Notice of Hearing is "ordering someone to appear", you may not use this form, you would need to fax or email the Notice and Order for the Judge's signature. If it does not indicate an order to appear, you may continue to use this form.**

  • Found an available date(s) on the Court Calendar?
  • Confirmed selected date(s) with the opposing counsel?

You may not use this form unless every attorney of record or unrepresented party has either email or fax capability.

Please Note: Dockets are run at 2 pm. If you are requesting a setting for tomorrow, your request MUST BE RECEIVED by 1:30 pm.
